Complex Care at Home

High-intensity clinical care delivered safely at home. Our nurse-led complex care teams support people with significant health needs to live outside hospital walls.

Complex health needs should not mean a life confined to hospital. With the right clinical team, equipment and planning, people with high-intensity care needs can live safely and well at home, closer to family and the life they know.

Our complex care service is led by registered nurses and delivered by support workers who have completed high-intensity support training. Every plan is built around detailed clinical protocols, with clear escalation pathways so everyone knows exactly what to do if something changes.

We support people with a wide range of needs, including ventilation, tracheostomy care, enteral feeding, catheter and bowel care, seizure management and complex medication regimes.
